UNDERTAKING FROM THE STUDENT

1. I hereby declare that the entries made by me in the Application-cum-Enrollment Form are complete and true to the best of my knowledge, belief, and information.

2. I hereby undertake to present the original documents for verification immediately upon demand by the concerned authorities of………………………………………………….

3. I hereby promise to abide by the rules and regulations concerning admission, attendance, discipline, etc. of………………………………………………………. and to follow the Code of Ethics prescribed for the students of………………………………………………. as in force from time to time, and any subsequent changes/modifications/amendments made thereto. I acknowledge that…………….......................................…………………. has the authority to take punitive action against me for violation or non-compliance of the same.

4. I have been informed at the time of admission that the fee paid by me is interim and subject to change/revision as per the decision of the Admission and Fee Regulatory Committee / Hon’ble Court / competent authority of the college/institution.

5. I hereby agree and undertake to pay the difference in fee, if any, as decided by the Admission and Fee Regulatory Committee / Hon’ble Court / competent authority of the college/institution.

6. I hereby declare that I will pay my annual tuition fee, hostel charges, and other charges every year from the 1st of February to the 28th of February, or with a late fee of Rs. 500 per day after the cut-off date for fee payment, as approved by the Admission and Fee Regulatory Committee.

7. I also understand that without payment of all fee dues, 80% attendance, and 50% internal marks, I will not be allowed to appear in the University examination.

8. I also agree that I will deposit all outstanding fee dues against me and obtain a No Dues Certificate before filling out the application form for the University examination.

9. I hereby declare that I will not indulge in, nor tolerate, ragging in any form, even in words or intentions, and I undertake to submit an undertaking in the prescribed format for the same.

10. I hereby acknowledge that I shall be liable for expulsion forthwith from………………………………………………………………………………………. on being found involved in, or committing, any offence cognizable and punishable under the Narcotic Drugs and Psychotropic Substances Act, 1985.

11. I also declare that I am not suffering from any serious/contagious medical ailment and/or any psychiatric disorder.

12. I further declare that my admission may be cancelled at any stage if I am found ineligible and/or if the information provided by me is found to be incorrect.

14. I will strictly adhere to the code of conduct as laid down by ………………………………………………………………………………………. I will not indulge in any act of indiscipline while inside or outside the college/institution campus / clinical settings / hostel premises.

15. I hereby declare that I will neither join any agitation/strike to force the authorities of the Institute to resolve any problem, nor will I participate in any activity that tends to disturb the peace and tranquility of the academic environment of the college/institution campus and/or its hostel premises.

16. I hereby declare that I shall be solely responsible for my involvement in any kind of unlawful activity, whether inside or outside the campus, and shall be liable for punishment as per the law of the land. I further understand that the Institute shall in no way provide any support to me and will not be held responsible for any such action.

17. I hereby undertake that if I am found involved in any of the following, I will have to pay the hostel fee and college/institution fee for the entire academic session of the admitted course/program without any objection, despite not being allowed to stay for the remaining period of the course, and the Institute shall be at liberty to take legal recourse (civil/criminal or both):

(a) Defacing / vandalizing hostel property

(b) Misbehaviour with teaching faculty / hostel staff / clinical staff

(c) Engaging in verbal abuse / physical assault / sexual harassment / criminal activity

(d) Possession of firearms / weapons

(e) Consumption / possession of any psychoactive substances (alcohol, cigarettes, cannabis, hallucinogens, cocaine, etc.)

(f) Cooking inside hostel rooms, borrowing utensils or food from the mess to the room, and using unauthorized electrical appliances in the hostel

(g) Withdrawal from college/institution or hostel on my own / parents’ decision after joining the college/institution
